Instructions for Mail Form

Print and fill out the form, attach a copy of your driver's license or state-issued ID card, $7.00 (money order or cashier's check only), and mail it to Anderson Police Department Records, 1040 Main St., Anderson, IN 46016

• COST: The cost of a limited criminal history is $7.00 and must be paid with a money order or cashier's check. Please include your payment method with your request.

• IDENTIFICATION: You must include a copy of your driver's license or state-issued ID card.
